Paul Dean Email: frpaulddean@gmail.com Mobile: 711 061 864 Should the report concern either the Safeguarding Officer or the Lead Chaplain directly then please only report to the other party. Note: confidentiality is of the utmost importance and concerns must not be discussed with third parties. 3. The Chaplaincy Safeguarding officer or the Lead Chaplain must complete the following: a) Safeguarding Concern Referral Form b) Details for Victim / Survivor c) Details of Respondent / Person of concern Available to download at – Diocese of Europe, Safeguarding Protocol and Guidance 2019 Safeguarding Forms Safeguarding Concern Referral Form These should be completed with the agreement of the Chaplaincy Safeguarding Officer and the Lead Chaplain as quickly as possible before submitting the forms to the Diocesan Safeguarding Team: europe.safeguarding@churchofengland.org 4.
